Additional Forms
Students have the option with each form of either filling out the online form or printing off the PDF version to fill out and mail to Anderson University.
Re-activate Form: A re-activate form is used for admitted students that decided not to attend AU at the time. The re-activate form saves an admitted student from having to go through the entire admissions process again.
Re-admission Form: A re-admission form is used for students that have matriculated into AU then left, but now want to come back to school. The re-admission form saves matriculated students from having to go through the entire admissions process again.
Athletic Questionnaire: Students interested in playing sports at Anderson University.
