A Healthy U Reward = $100.00 HSA Contribution each quarter
You must earn 50 points in a quarter (all or nothing)

IMPORTANT NOTE: The following table shows the time frame for each quarter and when the contribution will be deposited in your Health Savings Account.

1st	January 1 - March 31	April
2nd	April 1 - June 30	July
3rd	July 1 - September 30	October
4th	October 1 - December 31	January

2. Attend Seminars
3. Participate in Special Programs
